Programs & Accomplishments

In-Services: ETHAN hosts monthly in-services for consumers,
providers, agency personnel and Network members aimed at identifying
resources and increasing access to services on the first Thursday of
each month at 10:00 a.m. in the classroom of the Robert Hall
Administrative building, located behind CHRISTUS Jasper Memorial
Hospital in Jasper. Everyone is welcome to attend. |
| Information and Referral: ETHAN
receives more than 10,000 calls a year from area residents seeking
information on available programs and services. As a result of
ETHAN's collaboration with its community partners, ETHAN is able to
respond to these inquires or provide a referral to a community
partner that can. |

HELP: Health Equipment Loaner Program: Established with a generous donation from The
Jesse Tree of Galveston, and supported through donations from community
partners and area residents, ETHAN provides uninsured and
underinsured individuals with medical equipment and supplies at no
cost to the individual or family. Equipment inventory includes
hospital beds, walkers, wheelchairs, shower chairs, and many other
items. A prescription from a physician is required to obtain health
equipment. Availability of equipment is based on program inventory.. |
| Community Health Education:
ETHAN collaborates with
community partners throughout the year to offer free health education
classes on a variety of topics: Heart Disease, Stroke, High Blood
Pressure, Asthma, High Cholesterol, Diabetes and others. If your
church or organization would like to host a class, please call the
Network's main office. |

God's Fan Club: Provides
cooling equipment - small window air conditioning units and, or box
fans - to the elderly, homebound or the physically disabled that
live without the means to cool their residences. To qualify for a
window air conditioning unit an individual must have a prescription
from their physician for a respiratory related ailment and have no
other means with which to cool their home. Availability of cooling
equipment is based on program inventory. |
| Heart Throb Cafe: ETHAN's
Heart Throb Cafe' provides a variety of programs including cooking
classes, meal planning and instruction in preparing Heart Healthy
meals. Modifying recipes will allow a diabetic or person with high
blood pressure to still enjoy many of the foods they love. |

Essentials: A
program supported by community partners and area residents that
provide items essential to the overall health and well being of an
individual but are often not covered by Medicare or other health
insurance. These items are provided free of charge and include adult
diapers, blue pads, nutritional supplements, limited diabetic
testing supplies and other essential health items. To qualify, a
person must be un/underinsured for items requested. Availability of
supplies are based on program inventory. |
| Care Partners: ETHAN
Community Health Workers provide care management services to assist
individuals who use the ER as their main source of primary health
care in identifying permanent medical homes that will provide them
with low cost or no cost medical care. In addition, Community Health
Workers also provide culturally competent health education and
connect individuals to other health supporting services they are
eligible to receive. |

Healthy Smiles: Provides
low cost dental services to individuals that are uninsured for
dental care. Dental clinics are held monthly at different locations
in the Network's service area. Dental services are made possible
through a contract between ETHAN and the University of Texas Health
Science Center at Houston, Dental Branch. Dental services are
provided by appointment only and include dental exams, x-rays,
cleanings, extractions, and fillings. Fees are based on household
income. Call ETHAN's main office to schedule an appointment. |
| ETHAN Optical Shoppe: Provides
eye exams and eyeglasses to adults who cannot afford them. To
qualify for assistance through this program an individual must meet
income guidelines and be uninsured for vision coverage. Persons
approved for assistance through this program are required to
participate in the cost of the eye exams and eyeglasses through
payment of co-pays based on household income. |

Child Safety Seat Program: Provides
child safety seat classes and distributes free child safety seats to
low-income families in the Network's service area. Individuals
receiving care seats must attend a short 45-minute safety class.
Distribution of safety seats is based on program inventory. |
| Disease Case Management Program: Provides
uninsured individuals with one-on-one instruction and education in
managing their high blood pressure, diabetes, asthma and other
chronic conditions. To enroll in this program an individuals must be
uninsured and referred by a physician. Once enrolled individuals
must agree to commit to life style changes that will improve their
health conditions, such as learning to take their medications
correctly, eating right, exercising and other health enhancing
activities. |

Blood
Pressure, Diabetes and Cholesterol Clinics: Scheduled
several times throughout the year, ETHAN collaborates with community
partners to offer free blood pressure, diabetes and cholesterol
screenings to area residents. The early identification and treatment
of these conditions can greatly reduce an individual's risk for
heart disease, diabetes and its complications. |

RxMedConnection: Is
a prescription assistance program that assists individuals in obtaining
medication from the patient assistance program of the major
pharmaceutical manufacturer's.
Over $3 MILLION
in free prescription medications have been obtained for
qualified individuals in the Network's service area.
